How it works

How ReadyRoom works.

Your mom is coming home Wednesday. ReadyRoom helps you figure out what needs to happen before she gets there, what to ask, and what to do in the first days home.

ReadyRoom is a phone-based plan for families from before discharge through day 30. It opens in your web browser, so there's nothing to download.

The hospital prepares the discharge. ReadyRoom prepares the family.

Step by step

From the call to day 30.

What to notice. What to ask. What to do next.

  1. When the call comes

    Answer a few questions

About three minutes. Nothing from their chart.

  2. Right after

    See your first step

Picked from what worries you most.

  3. Before leaving the hospital

    Ask the team

The words to say, written out.

  4. The first 72 hours

    Settle in at home

Medicines, the call card, the four basics.

  5. This week

    Take the next steps

Week by week, in any order.

  6. Day 30

    See what you set up

Keep it, hand it off, or call it done.

What it looks like

One daughter, one phone, one week.

  1. The call

Her father is in the hospital. She is four hours away and at work. That evening she opens ReadyRoom on her phone.

  2. Three minutes

A few questions, one at a time. Where things stand. Who is nearby. Whether she has the medicine lists. What she is most worried about. No name, no diagnosis, nothing from his chart.

  3. Before he leaves

She has the questions to ask the team, and the words to say, written out. She reads them down the phone to her brother, who is the one standing in the room.

  4. The day he comes home

The plan opens on the first days home: what to do today, and what can wait. Her brother checks the four basics first: a clear path, a light at night, rugs that stay put, something steady by the toilet.

  5. Days 2 and 3

Now the rooms. She is there, standing in his room with her phone out. ReadyRoom walks the bedroom, the bathroom and the stairs, one room at a time. What to move. What to order.

Who it's for

Opening first, and what comes later.

Opening first

For a hospital stay happening now or just behind you: the plan from before discharge through day 30.

Not the right fit yet

  • Nothing has happened yet and you are planning ahead. That version is being built: join the waitlist, and we'll tell you when it's ready.
  • There was a fall or a near miss, but no hospital stay. That part comes next.
  • You are looking for advice about a diagnosis or a medicine. That belongs with their clinician.

ReadyRoom is educational guidance, not medical advice. Medical questions go to the care team.

Your next step

Know what comes next.

We'll email you when ReadyRoom opens.
